The Association between Obesity, Age and Prostate-specific Antigen in Adults

Abstract
Purpose: The association between obesity and prostate-specific antigen(PSA) in men has yet to be clarified. We carried out this study to investigate the association between age, obesity index and PSA in Korean men over 40 years old.

Materials and Methods : From April 2004 to April 2005, 1,147 men over 40 years old who visited our Heath Promotion Center were enrolled. The medical history were obtained through history taking and questionnaires. Physical parameters such as height, weight and waist and hip circumferences were measured and body fat(%) was checked using body composition analyzer. We checked serum chemical tests including lipid profiles, liver and renal function test, PSA and urinalysis.

Results: According to body mass index(BMI), 496 males were classified as obese(BMI£¾25kg/m2) and 651 as normal group. In the normal group, men 65 years old or older had higher log PSA than men younger than 65 years(p=0.002), but in the obese group, there was no such difference according to age. In multi-logistic regression analysis controlling life style-related parameters such as smoking, drinking or regular exercise, the log PSA increased by 0.010 for an increase of 1 year. But, obese group showed less increase of log PSA than normal group.

Conclusions : Obesity can intervene in the relationship between serum PSA and age. Slower increase of serum PSA per year in obese men over 65 years old than in normal men might suggest that it takes more time for serum PSA to reach its threshold for prostatic biopsy in this group. This result might suggest that prostate cancer in this group would be found in a more advanced state, causing a negative impact on treatment outcomes or prognosis of these men.
